Pathways Permanent Supportive Housing

Pathways Permanent Supportive Housing provides permanent housing with wraparound services for chronically homeless individuals with disabilities using the Housing First model. We believe everyone deserves a home regardless of sobriety, mental health status, or criminal history. Residents sign standard leases paying affordable rent while receiving intensive case management. Our teams include nurses, therapists, and social workers addressing complex medical and mental health needs. We help residents access disability benefits and healthcare improving health while maintaining housing. Our approach has proven highly effective with ninety percent of residents remaining stably housed. We advocate for expanding affordable housing and supportive services recognizing housing as fundamental human right.
