Shelter Plus Care Supportive Services

Shelter Plus Care Supportive Services combines permanent housing assistance with intensive case management for homeless individuals with serious mental illness and substance use disorders. Our program serves the most vulnerable homeless population requiring highest level of support. We provide housing subsidies ensuring clients pay only thirty percent of income toward rent. Our clinical team includes psychiatrists, therapists, and certified peer specialists delivering evidence-based treatment. We use trauma-informed care recognizing impact of homelessness on mental health. Our assertive outreach meets clients wherever they are building trust before discussing housing. We coordinate with hospitals and jails ensuring smooth transitions to housing and treatment. Our commitment continues as long as clients need support.
